Friends, Former Patients Celebrate 90Th Birthday For Physician Dr. Rei Ozaki

The surprise celebration was a bit overwhelming for the 90-year-old doctor, a Downey pioneer who came to the city almost 60 years ago as a young doctor eager to help his patients.  But the smile on his face when he walked in the restaurant and looked around the room said it all.

Back in the 1950s, from his office on 3rd Street in the city of Downey, Dr. Ozaki delivered babies, tended to the old and infirmed and dedicated his professional life to caring for hundreds of Downey families. Ozaki attended Loma Linda University, where he completed his medical education and received formal training as a family doctor who delivered almost all of the care to his patients.

Each morning, he’d arrive at Downey Regional Hospital to perform surgery and then spent up to 10 hour days in his office seeing patients and finally returned to the hospital in the evening to care for and visit patients.

He made house calls and treated countless patients who did not have the money to pay him or health insurance.

“It was a time in healthcare when family doctors like myself did everything in medicine for our patients, from delivering their babies to caring for their grandparents and doing surgery,” he recalled.  “If I fit the bill for an old fashioned doctor, then so be it – that’s what I was and continued to be for decades.”

Today, at age 90, Dr. Ozaki still works, getting up each day to help Dr. Edelstein, a Downey eye physician. “I don’t have a practice any more but I still keep my fingers in the medical arena by helping my fellow colleagues,” he said. 

His work and his love of his rose garden seem to keep a twinkle in his eye.  “I’d still be seeing my patients if I could,” he added.  “I miss them terribly.”

It is not uncommon for Dr. Ozaki to be greeted by his former patients, or their children or grand children, all of whom he treated as a family physician.

“He is an amazing doctor, the kind of doctor whose spirit for caring for patients was contagious to all of us,” recalled Sheldon S. Zinberg, MD, a long-time Downey physician who founded CareMore and worked along side Dr. Ozaki. “He never took a vacation or days off. He committed his life to making sure his patients were well and healthy. “

Today, as he battles the aches and pains that come with living to be age 90, he still can be found on the computer in Dr. Edelstein’s office, researching new advances in medicine.

“Medical science still amazes me, and it keeps my mind engaged,” he said. Sounds like just what the doctor ordered.

Physician Shortage Growing, But Not From Health Care Reform

Last week, we posted a very interesting article from the New York Times regarding the physician shortage, and how it’s about to get much worse. You’re probably thinking that this impending physician shortage is due to the passing of health care reform, but you’d be surprise to know that much of it has nothing to do with health care reform. Concierge medicine may be more of an option than you ever realized.

 

As recently as 2005, more than two-thirds of medical practices were physician-owned — a share that had been relatively constant for many years, the Medical Group Management Association says. But within three years, that share dropped below 50 percent, and analysts say the slide has continued. (NYTimes)

 

Medical students are not only opting out of primary care and turning to specialized practices, the very few that are choosing primary care are graduating with so much student debt that they have no choice but to accepted salaried positions at hospitals and health systems. There is no longer any incentive for students to open, or even work in private practices.

 

Physician’s wages are decreasing while their overhead is going up. Their hours are getting longer while their families are suffering. Patients are getting ‘assembly line’ treatment and diagnosis instead of proactive, preventative health care because primary physicians are taking on more and more of them to cover costs. Where does it end?

According to The New York Times article, patients are losing the “intimacy of longstanding doctor-patient relationships” when so many doctors are choosing salary over care. It’s such juxtaposition, an ironic reality, where graduating students are turning to salaried positions where they can make more money and spending less time with patients, and are respected for their choices. However, should a family physician who has been practicing for years decide to convert to a concierge practice so he can become a more effective doctor, spend more time providing affordable preventative medicine that takes out the big middle man (insurance companies), he is frowned upon.

 

Perhaps one day we’ll dive into the psychology behind a patient’s reactions to concierge medicine. It would be an interesting study to understand why this form of medicine insights such interesting responses. But I digress.

 

What’s interesting about the decline in private practices is that they just aren’t equipped to handle the changes that health care needs to be making. For example, take electronic health records. They will be required now that health care reform has passed, but private practices just don’t have the time, money nor resources to convert their practices electronically.  These computer systems are extremely expensive and incredibly time consuming to set-up. Imagine transferring every piece of paperwork in your entire office to digital. It would take a lifetime. Small practices cannot compete with large organizations that have people who do nothing else but transfer data.

 

For older doctors, the change away from private practice can be wrenching, and they are often puzzled by younger doctors’ embrace of salaried positions.

 

“When I was young, you didn’t blink an eye at being on call all the time, going to the hospital, being up all night,” said Dr. Gordon Hughes, chairman of the board of trustees for the Indiana State Medical Association. “But the young people coming out of training now don’t want to do much call and don’t want the risk of buying into a practice, but they still want a good lifestyle and a big salary. You can’t have it both ways.”

 

Concierge physicians kind of do have it both ways. This is what’s interesting about this entire discussion, because by converting to a concierge practice, your salary improves, your patient-doctor relationship improves due to your 24-7 availability and on-call status, your quality of care improves, and overall things are pretty good.

Family Practice Coding: For Complete Patch Test Coding, Count Every Patch And Office Visit

When your family physician carries out a patch test and follow-up for a patient, do not let some of the calculations go unnoticed. Count each patch and any extra evaluation and management services to round out a complete claim and see your bottom line soar by about per patch.

A new patient visits your office with a red, itchy rash on his arm. The family physician makes an initial diagnosis of non-specified contact dermatitis (692.9, Contact dermatitis and other eczema; unspecified cause). Then the doctor applies patch tests and enquires the patient to come back for readings in 48, 72, and 96 hours.

Report it: Since your doctor applied the patch test, you will report 95044. You will also bill for any evaluation & management service the family practitioner provided for the patient. Since you are filling a claim for a new patient, choose the proper evaluation & management code from 99201-99205 (Office or other outpatient visit for the E&M of a new patient …).

Change it: Add modifier 25 (Significant, separately identifiable E&M service by the same doctor on the same day of the procedure or other service) to your evaluation & management code to notify the payer that the physician performed an initial evaluation that led her to complete patch testing on the patient. You can also think about reporting modifier 25 while coding an E/M service.

While conducting a patch test, the doctor applies various patches on the patient to test for his reaction to various allergens. Payers take each test as an individual procedure; therefore you should calculate accordingly when billing. Even though 95044 carries only 0.18 total RVUs, the national average Medicare facility and non-facility fee is .12 (based on the national conversion factor of 33.9764). Your payment can grow considerably depending on the number of patches applied.

Bill your units in block 24G of the CMS-1500 form as per the number of allergens tested. Teach your staff to double check the number of units on the charge ticket, and educate your physician to document the correct units. Front-desk staff can also aid by asking the family physician how many patch tests she administered.

Just as you report 95044 for each patch test applied, you will submit an evaluation & management code for each follow-up visit. In the above instance, the patient returns to the office in 48, 72, and 96 hours. Each of those visits will be coded using an evaluation & management code. Since the doctor is now seeing an established patient, choose your evaluation & management code from 99211-99215 (Office or other outpatient visit for the evaluation and management of an established patient …).

Exception: If a nurse reads the patch tests, stay away from higher-level E/M codes. Family physician is onsite when the staff member reads the results. The doctor should be able to make a more definitive diagnosis once she reads the patch test results.
